A custodial care facility providing supportive and personal care services to disabled and/or elderly individuals who cannot function independently in most areas of activity and need assistance and monitoring to enable them to remain in a home like environment.

Adult Living Care Facility 

A residential care facility that provides support to people who can't safely live by themselves but who don't need skilled medical services 24 hours a day, and thus don't need to be in a nursing home, where more intensive care is provided. 

Non-emergency Medical Transport 

Provides transportation services to patients who face extraordinary barriers getting to their medical appointments. 

Adult Day Support

Includes services and supports that enhance skills, community membership, and independence, as well as personal choice, all in order to attain or maintain maximum potential and community engagement.
